Supervising the Implementation of ESFT

This workshop identifies common challenges new and experienced supervisors face in helping intensive in-home therapists implement ESFT with fidelity.  Issues are addressed related to transitioning from other more traditional treatment models to a ESFT supervisor role.  The ESFT supervisor’s roles and responsibilities are discussed and clarified.  As a way to become more knowledgeable of the ESFT model, videotapes of family sessions are reviewed  and rated with the ESFT Treatment Adherence Scale and the ESFT Clinical Competencies Scale.

This is a live synchronous distance learning activity conducted in real time, allowing for simultaneous participation of participants and instructors from different locations.

Objectives:

As a result of participating in the forums, participants will be able to:

  1. Identify common challenges in supervising implementation of  ESFT
  2. Accurately recognize clinical competencies and ESFT treatment processes in videotapes of family therapy sessions.

In ESFT, it is assumed that children’s presenting problems are best understood and treated in the context of their family’s structure.  Enmeshment is one of four structural patterns most associated with serious emotional disturbance in children. This workshop describes the characteristics of enmeshment and how this interactional pattern shapes children’s development of social-emotional competencies and their response to treatment. Treatment guidelines are highlighted, as well as common clinical traps therapists are likely to encounter when working with enmeshed relational patterns.

Objectives 

As a result of attending this training, participants will be able to:

  1. Recognize the markers of enmeshment in families.
  2. Describe how enmeshment impacts youth symptom development.
  3. Recognize four maladaptive strategies used to minimize conflict in enmeshed families.
  4. Identify the targets of change when working with enmeshment.
  5. Describe common clinical traps when working with enmeshed systems.

Understanding and Treating Stepfamilies: A Subsystem–Based Approach

This workshop will present a practical approach for understanding and treating stepfamilies.  Both living in and working with step-families can be complicated and challenging at times.  Stepfamily Therapy (Sft), an evidence-based model, was co-developed by Dr. Browning. It is an approach that has been outlined in his co-authored book from APA Books.  This workshop will highlight common mistakes of family therapists who work with stepfamilies and present ways for helping professionals to avoid them.

As a result of participating in this training, attendees will be able to:

  1. Recognize the unique needs of stepfamilies;
  2. Observe demonstrations of treatment with stepfamilies;
  3. Recognize family therapy techniques not suited for stepfamilies;
  4. Practice specific techniques that work with stepfamilies;

About The Trainer
Scott W. Browning, Ph.D. is a professor emeritus in the Department of Professional Psychology at Chestnut Hill College in Philadelphia.  He is a noted authority on psychological treatment with stepfamilies.  As a scholar, teacher and clinician, Scott has explored the intricacies of treating stepfamilies, and has provided advanced training in the treatment of stepfamilies to clinicians and graduate students both nationally and abroad.

In ESFT, enactments are used to re-shape family negative interactional patterns (NIP). They are the ultimate tool for the family-based therapist. To make the experience meaningful and the changes sustainable it is imperative for the family members to be central and active in both the development and the process of the intervention. It is also crucial that the goal of the enactment be laser-focused and clearly defined. In today’s workshop, we will review and discuss the phases of enactment from an advanced ESFT therapists’ perspective. Interaction among participants will be facilitated around observations of a videotaped high-quality enactment from a family therapy session. The videotape is used to demonstrate both laser-focus and the family’s centrality in the enactment. Participants will be encouraged to discuss the strengths and challenges they have faced when conducting enactments and lessons learned from their years as a therapist in FBMHS.   

This workshop includes lecture, video-tape review, handouts, and breakout rooms for discussion and for practicing skills. Participants will be provided with a comprehensive list of resources for further study.

Objectives 

As a result of attending this training, participants will be able to:

  1. Describe the phases of enactment (Preparation, Initiation, Facilitation, Meaning-Making
  2. Identify challenges and successes with keeping sessions present focused using enactments
  3. Identify three strategies that can be used to keep the interaction going in the facilitation phase of enactment. 

This is an intermediate level course. The target audience is behavioral health professionals who have completed the three-year training within Pennsylvania’s Family Based program. This workshop introduces the ESFT theory of change, a session-level description of actions that therapists take to promote change. A theory of change is explained, highlighting specific therapist actions directly linked to treatment outcomes in ESFT-FBMHS. This workshop addresses the question often asked by therapists in ESFT training, “What do I do in sessions to promote change?”  Seven ESFT mechanisms of change and the actions associated with each are described in detail and demonstrated through videotapes of family therapy sessions.  Participants will practice using an ESFT fidelity self-monitoring tool with videotaped family therapy sessions to identify therapists’ actions consistent with ESFT mechanisms of change.

Objectives 

As a result of attending this training, participants will be able to:

  1. Describe how to conduct a family session using ESFT.
  2. Explain the nature of change mechanisms in treatment.
  3. Describe seven core ESFT mechanisms of change.
  4. Recognize the presence or absence of ESFT mechanisms of change in videotaped family therapy sessions.
