This workshop identifies common challenges new and experienced supervisors face in helping intensive in-home therapists implement ESFT with fidelity. Issues are addressed related to transitioning from other more traditional treatment models to a ESFT supervisor role. The ESFT supervisor’s roles and responsibilities are discussed and clarified. As a way to become more knowledgeable of the ESFT model, videotapes of family sessions are reviewed and rated with the ESFT Treatment Adherence Scale and the ESFT Clinical Competencies Scale.

In ESFT, it is assumed that children’s presenting problems are best understood and treated in the context of their family’s structure. Enmeshment is one of four structural patterns most associated with serious emotional disturbance in children. This workshop describes the characteristics of enmeshment and how this interactional pattern shapes children’s development of social-emotional competencies and their response to treatment. Treatment guidelines are highlighted, as well as common clinical traps therapists are likely to encounter when working with enmeshed relational patterns.
Objectives
As a result of attending this training, participants will be able to:
- Recognize the markers of enmeshment in families.
- Describe how enmeshment impacts youth symptom development.
- Recognize four maladaptive strategies used to minimize conflict in enmeshed families.
- Identify the targets of change when working with enmeshment.
- Describe common clinical traps when working with enmeshed systems.

In behavioral health agencies providing intensive in-home treatment, there is often little organized preparation for stepping into a supervisory position overseeing the implementation of ESFT. Initially the role can be overwhelming, given the multiple tasks of monitoring fidelity for model implementation, supporting therapists’ clinical skill development, and engaging reluctant staff who may have been practicing a different model before ESFT.
This workshop clarifies what effective ESFT supervisors do supervision. Strategies are presented for preparing the ESFT supervisor for the role through organized onboarding procedures that cover common challenges. As part of this goal, experienced ESFT supervisors are encouraged to share “lessons learned” with the newer ESFT supervisors. Focus is given to how new supervisors can best incorporate CFBT self-monitoring tools such as Recognizing Process in Clinical Supervision Rating Form and the Doing of Supervision Form. Videotapes of family therapy sessions and supervision sessions are reviewed for practicing using these supervisory tools.

This workshop introduces the ESFT theory of change, a session-level description of actions therapists takes to promote change. A theory of change is explained, highlighting specific therapist actions believed to be directly-linked to treatment outcomes. This workshop addresses the question often asked by therapists new to ESFT, “What do I do in sessions to promote change?” Seven ESFT mechanisms of change and the actions associated with each are described in detail and demonstrated through videotapes of family therapy sessions. Participants will practice using ESFT fidelity measures to identify therapists’ actions consistent with ESFT mechanisms of change.
Objectives
As a result of attending this training, participants will be able to:
- Describe how to conduct a family session using ESFT
- Explain the nature of change mechanisms in treatment.
- Describe seven core ESFT mechanisms of change.
- Recognize the presence or absence of ESFT mechanisms of change in videotaped family therapy sessions.

This series counts toward required annual training hours in Family Based Mental Health Services, but is not currently available for CE credit.
One of the challenges in both training and supervision is choosing effective methods for developing clinicians’ EcoSystemic case conceptualization. This workshop gives focus to a brief supervision tool, the DOC-F, for reviewing supervisee case write-ups and providing feedback on the essential skills related to ESFT case conceptualization. A brief overview and explanation of each of the items on the DOC-F is provided. Excerpts from case conceptualizations are used as opportunities for supervisors to practice DOC-F ratings and to promote inter-rater reliability. The DOC-F is discussed in the context of avoiding some of the more common errors in supervision.
Objectives:
As a result of attending this educational activity, participants will be able to:
- Describe three common mistakes in clinical supervision
- Describe the rationale for the DOC-F and how use it to enhance EcoSystemic thinking
- Use the DOC-F to practice rating case conceptualizations of supervisees.
